Applications
How to Apply
- Download, print (single-sided), and fill out the Program Selection form and Common Application forms. DO NOT complete these forms electronically, print the forms double-sided, or staple the forms.
- Ensure that all portions of the form have been filled out. There are some areas of the forms that are commonly missed by parent(s)/guardian(s) which include 1) writing the name of the participant at the top of the Participation Agreement and 2) if there are two parent(s)/guardian(s), including the signatures/initial of both parent(s)/guardian(s) on all areas such as the Assumption of Risk forms and 3) filling out the details in the bottom half of the Media Release form.
- Scan all forms (Program Selection and Common Application forms) as a single PDF and upload to the online portal.
- Complete payment upon request of staff. Staff will review your application electronically and send you the link to complete payment. Please allow for 5-7 business days to receive this email from staff.
- Deliver the fully and correctly filled out forms to John Meany either upon arrival at registration or by mailing them to arrive prior to the program start date. If you are mailing the forms in, you will receive the mailing address via email upon review of your forms.
Deadline
